To make check-in during your first visit easier:

Create an account and log-in as a site member and complete the registration form below.  

Upload vaccination records so we can ensure your dog is up to date on the following required vaccinations:

Rabies, Distemper (DHPP, DHLPP, or DA2PP) and Bordetella.

If you do not have an electronic copy of your records, please take a picture with your phone and upload here.

Please ensure all records show your dog's name and expiration date of the aforementioned vaccinations. 

Your Dog must also be up to date on Flea/tick prevention and have a negative fecal to play at the park. 

Upload those records with proof of vaccination.

Not up to date?  No problem!  Make an appointment with one of our one site Doctors and get your Dog updated to play!
